Operate or not?

Can I paddle with it if I have to delay surgery for a while or will this damage the joint further?

I would let pain be yr measure, it does dep how slack your shoulder is, and whether the labrum is moving or being pinched when your paddling. They don't always operate, again dep on how damaged it is.
